The following information relates to the patient with cancer who is entering the end of life phase of their illness, however it is recognized that many of the principles are applicable in other diseases. Definitions in palliative care are not always clear, however for the purpose of this chapter we will consider those patients who have weeks, days or hours to live while considering some of the issues that should be addressed before this period such as advance care planning and co‐ordination of care. To offer some clarity, definitions have been provided, as follows.
